高雄長庚醫院院長陳肇隆指出，活體肝臟移植，肝臟大小只需受肝者體重的百分之零點八。如果從捐肝者身上摘取的肝臟最小單位（通常是左外側肝葉），仍大於換肝者需求，勉強植入就可能產生「大肝症候群」，除會頂到橫膈膜，造成受肝者呼吸困難，還可能壓迫到剛重建好的肝靜脈、門靜脈，阻礙血流通暢，影響肝臟功能，甚至死亡。

因此碰到這種情況時，即需以「超減容積」技術處理，手術主要是在分切肝臟時，必須保留肝臟上頭的血管、膽管完整，在移植時不致影響相關功能。
